Summary of the project
JHF is committed to strengthening end-of-life communication among providers, patients, and their loved ones, and ensuring that everyone has the information and resources needed to make informed decisions in the event of a life-limiting illness.
As the coordinator for PA POLST, JHF offers education and resources to patients, families, and their healthcare providers across the state. Since 2010, the Foundation has engaged nearly 900 healthcare professionals through PA POLST train-the -trainer courses
