RAVEN

The Reduce Avoidable hospitalizations using Evidence-based interventions for Nursing facilities in Western Pennsylvania (RAVEN) Project is a four-year, UPMC Aging Institute-led grant funded by the Centers for Medicare and Medicaid Services. JHF is one of four Operating Partners in the grant (along with Robert Morris University, Heritage Valley Health System, and Excela Health). The Project is focused on using evidence-based interventions at nineteen skilled nursing facilities in western Pennsylvania to: (1) reduce the frequency of avoidable hospital admissions and readmissions among nursing facility residents; (2) improve nursing facility resident health outcomes; (3) improve the bi-directional process of transitional care between hospitals and nursing facilities; and (4) reduce overall healthcare spending without restricting access to care or choice of providers. Among other things, JHF provides training and coaching covering a variety of topics including quality improvement, palliative care, communication and data/metrics.
